Alfredius is a scientist and doctor in Yharnam. When Ashen Blood struck Old Yharnam, he started studying its pattern. Later on, he teamed up with Priest Clement, a Healing Church member to uncover the disease origins. They started exchanging samples and knowledge revolving the disease and its origins, and eventually become good friends. When Mathilde, a neighbor and good friend of Alfredius succumbed to Ashen Blood, he nursed her and even went as far as using the Old Blood given to him by Clement to treat her disease. Alfredius also tested Old Blood on himself on small doses for weeks, believing Old Blood, if applied low and infrequent enough, would heal the disease and the beastly curse would not manifest. After constant treatment, Mathilde's condition slowly betters and she recalls that she met her husband in her dreams who tells her that the "road is best taken lightly". Although she initially assumed he was referring to death, she believes he meant the life that awaits her. After not hearing from Clement for days and needing more of the Old Blood, he believes he has been betrayed and decides to give Clement up to church in search of more of it. Alfredius then spends his time catering for Mathilde as the city of Yharnam begins decaying from the plague. In subsequent days, Clement arrives at his doorstep with samples, citing that friends must stand by each other despite the risks. While the two embrace, several hunters standing behind Alfredius, with Alfredius sorrowfully revealing that Clement of his betrayal. As Clement is taken away by the hunters, the doctor regrets his decisions but finds solace in Mathilde's imminent survival thanks to the treatment. Some time after Clement execution, a ghoulish looking Mathilde slowly approaches Alfredius from behind, citing that she has been "healed".
